Stage 1 shows chakra and widens view. Stage 2 is Rotation. Stage 3 is 64 Palms. Stage 4 shows health and chakra, drains chakra on M1, and enlarges the palms hitbox. Stage 5 is Hyuga-only. Otsutsuki stops at stage 4. I do not have the written unlock for stages 4 and 5 beyond the clan gate.
Roll chance is 2 percent. Both tier lists say S. Member caps and a clan symbol are not on the card.

64 Palms, which the card describes as locking onto a target. That is not a general lock-on key for the whole game.
